1. Data Controller

Private Adıyaman Park Hospital San. ve Tic. A.Ş. ("Adıyaman Park Hospital"), we act as a data controller within the scope of the Personal Data Protection Law No. 6698 ("KVKK"). Your personal data are processed by our company for the following purposes and methods within the framework of KVKK and relevant legislation.

2. Purposes of Processing Personal Data

Your personal data collected are processed for the fulfilment of the services provided by our hospital and for the following purposes:

• Assessment of the patient and determination of the treatment plan,

• Provision of outpatient or inpatient treatment services,

• Carrying out surgery, medical examination, laboratory and imaging activities,

• Organising patient-specific catering and cleaning services,

• Making appointment reminders and other notifications,

• Carrying out accounting, financial management and archiving processes,

• Planning of interactions within the hospital,

• Evaluating patient feedback and communicating when necessary,

• Fulfilment of legal obligations.

3. Transfer of Personal Data

Your personal data may be shared with the following parties within the relevant legal framework:

• Suppliers,

• Authorised public institutions and organisations,

• Contracted private insurance companies and intermediary institutions,

• Personal or organisations that you have specified with your explicit consent.

4. Method of Collection of Personal Data and Legal Basis

Your data may be collected physically and digitally through the following channels:

• Hospital systems and portals,

• Telephone, e-mail and fax,

• Face-to-face meetings and courier/mail services.

Your data are processed on the basis of Articles 5 and 6 of the KVKK, performance of the contract, fulfilment of legal obligations, legitimate interests or explicit consent.

5. Rights of Data Subjects

Pursuant to Article 11 of the LPPD, you have the following rights:

• To learn whether your personal data is being processed,

• To request the purpose of processing your data and whether it is used in accordance with its purpose,

• Request correction of your incorrect or incompletely processed data,

• Request deletion or destruction of your data,

• If you believe that you have suffered damage as a result of a transaction contrary to your legal rights, request compensation for the damage.

You can submit your requests via our hospital's website or by applying in person. Your requests will be processed within 30 days at the latest, and in cases where a transaction fee is required, the cost will be reflected according to the relevant tariff.
